The primary purpose for the collection of the information on this form is to nominate exceptional law enforcement officers for the Third Annual U.S. Attorney General’s Award for Distinguished Service in Policing. The authority by which information is collected on this website form is 5 U.S.C. § 301 and 44 U.S.C. § 3101, as well as the Violent Crime Control and Law Enforcement Act of 1994, Pub. L. No. 103-322, 108 Stat. 1796 (1994). The information collected will be used by the Office of Community Oriented Policing Services (COPS Office) personnel or consultants, and personnel from other DOJ components, who will assist with selecting, vetting, and notifying the successful candidates for the Third Annual U.S. Attorney General’s Award for Distinguished Service in Policing. 

Any information you provide may also be disclosed pursuant to a “routine use” under the Privacy Act of 1974, 5 U.S.C. § 552a, listed in a DOJ System of Records Notice, which includes: JUSTICE/DOJ-003, Correspondence Management Systems (CMS) for the Department of Justice, https://www.gpo.gov/fdsys/pkg/FR-2001-06-04/pdf/01-13860.pdf. Your disclosure of information to the COPS Office on this form is voluntary.
